What data does CB-CAP collect?

CB-CAP collects minimal data: first name, county, services requested, and contact preferences. We do NOT collect Social Security Numbers, full addresses, medical records, diagnoses, or other Protected Health Information (PHI).

How much does CB-CAP cost for counties?

For founding partners (Schoharie & Delaware), CB-CAP is provided at no cost as part of The SozoRock Foundation grant program. Future counties will have a tiered pricing model based on population and services. Contact us for a customized proposal.

Can CB-CAP integrate with our existing systems?

Yes. CB-CAP has APIs to connect with Electronic Health Records (EHRs), county social services databases, and other platforms. Integration is customized to your county needs and existing infrastructure.

What training is required for county staff?

Navigators receive 2-day initial training plus ongoing support. County administrators receive dashboard training (4 hours). All training is provided by The SozoRock Foundation at no additional cost.

How long does implementation take?

From partnership agreement to launch: typically 3-6 months. Timeline includes infrastructure setup, staff training, community outreach, and pilot testing before full launch.
