Service Description

The Portal provides health navigation and resource connection services. We do not provide medical care, diagnosis, or treatment. Telehealth visits occur on provider platforms under the provider's responsibility.

User Conduct

You agree to provide accurate information and use the Portal for its intended purpose of accessing health equity services.
