Our Story

Building a movement for rural health equity.

The Founding

The SozoRock Foundation was established in 2025 to address a critical gap: underserved communities across America—particularly in rural areas—face severe healthcare access challenges, yet most health equity programs focus on urban centers. We believe residents in every community deserve the same quality of care and support, regardless of where they live.

Our Approach

Rather than building new clinics or providing clinical care, we focus on navigation—helping people understand and access the healthcare system they already have. By placing navigators in trusted community spaces like libraries, we remove barriers of transportation, stigma, and complexity.

The Innovation

The SozoRock Health Portal represents a new model: technology-enabled, community-embedded health equity infrastructure designed specifically for underserved communities. The strategic vision, technical architecture, and platform design were developed by Oluwabiyi Adeyemo, Director of Strategic Initiatives, who architected the systems and partnerships that make this model scalable and sustainable. We are proof that innovative solutions can thrive in places others have overlooked.

Looking Ahead

Starting with Schoharie and Delaware Counties in New York, we aim to expand across underserved communities nationwide—from rural towns to urban neighborhoods facing similar barriers. Every community we serve informs our model, making it stronger and more adaptable for the next.
