A Community-Based Health Equity Hub (CB-HEH) brings health navigation services directly to places you already visit — faith centers, schools, community halls, and county buildings. Our trained navigators set up at these locations on a scheduled basis to help you access benefits, prepare for telehealth visits, and connect with resources.
No. All services at Community-Based Health Equity Hubs are completely free. The SozoRock Foundation is a 501(c)(3) nonprofit, and our mission is to remove barriers to health access — including financial ones.
No documentation is required to get started. If you want help with benefits enrollment, having a photo ID and proof of income can be helpful, but our navigators can assist you regardless.
All services are available in English and Spanish. We are working to add additional languages based on community need.
Community-Based Hubs are mobile — they come to where you already are, rather than requiring you to visit a specific location. Library Hubs are permanent installations at partner libraries with regular hours.
No. We do not provide medical care, diagnosis, or treatment. Our role is to help you navigate the healthcare system, connect with providers, and access benefits. All medical care happens through your chosen provider on their platform.

Register on our portal and select "Community-Based Hub" as your preferred access model. We will notify you when hubs are scheduled in your area. You can also check our Schoharie County and Delaware County pages for upcoming locations.
