Eligibility Criteria

The Home-Based Hub is designed for residents who face significant barriers to visiting in-person hubs. Eligibility is assessed individually.

Mobility limitations that make travel difficult
No reliable transportation to library or community hubs
Homebound due to chronic illness or disability
Elderly residents with limited mobility
Caregivers who cannot leave home
Rural residents more than 30 minutes from a hub location

Meeting one or more criteria does not guarantee eligibility. Final determination is made during the phone consultation.

Register Interest

Register on our portal and select "Home-Based Hub." Answer a few questions about your situation.

Phone Consultation

A Health Navigator calls to understand your needs and assess eligibility. Takes about 10 minutes.

Tablet Delivery

If eligible, we deliver a pre-configured SozoRock® Tablet to your home with a simple setup guide.

Guided Setup

Your navigator walks you through the tablet by phone. You're ready to connect in about 15 minutes.

The Home-Based Hub is a pilot program. Eligible residents may receive access to a pre-configured SozoRock® Tablet for health navigation. The tablet remains the property of The SozoRock Foundation and must be returned per program terms. Eligibility requirements and program availability apply. Contact a Health Navigator for details.

No. The tablet includes built-in cellular connectivity — it works like a phone and connects to the internet via cell towers. As long as you have basic cell phone coverage at your location, the tablet will work.

Tablet loan periods are determined based on program availability and individual eligibility. We conduct periodic check-ins to ensure the program continues to meet your needs. Terms may vary as the program evolves.

Your Health Navigator is available by phone during business hours to help with any questions. We also provide a printed quick-start guide with large text and clear instructions.

The tablet is configured specifically for health navigation and telehealth. It includes a web browser for general health information, but is not intended as a general-purpose personal device.

Eligibility is assessed on a case-by-case basis during the phone consultation. Generally, residents who face significant barriers to visiting a Library or Community Hub may qualify — including those with mobility limitations, lack of transportation, or homebound status. Availability is limited, and the Foundation reserves the right to determine eligibility criteria.

None. The tablet does not store any medical records, diagnoses, or health information. All telehealth visits happen through your provider's platform. We never see or store your medical data.

Contact your Health Navigator. Replacement or repair is evaluated on a case-by-case basis and is subject to program terms and availability. Residents are responsible for reasonable care of the tablet.
